Abstract

BACKGROUND:

There has been serious concern regarding the longevity and durability of outcomes of reverse total shoulder arthroplasty (RTSA) in younger patients. It was the purpose of this study to analyze long-term outcomes and complications of RTSA for irreparable rotator cuff tears in patients younger than 60 years.

CONCLUSIONS:

RTSA in patients younger than 60 years leads to substantial subjective and functional improvement without clinical deterioration beyond 10 years. It is associated with a substantial complication rate, and complications compromise ultimate subjective and objective outcomes.

ANTECEDENTES:
Ha habido una gran preocupación con respecto a la longevidad y la durabilidad de los resultados de la artroplastia total de hombro reversa (RTSA) en pacientes más jóvenes. El objetivo de este estudio fue analizar los resultados a largo plazo y las complicaciones de RTSA para los desgarros irreparables del manguito rotador en pacientes menores de 60 años.

CONCLUSIONES:
La RTSA en pacientes menores de 60 años conduce a una mejora sustancial subjetiva y funcional sin deterioro clínico más allá de los 10 años. Se asocia con una tasa de complicaciones considerable, y las complicaciones comprometen los últimos resultados subjetivos y objetivos.

Abstract

BACKGROUND:

Although recent evidence suggests that any prior shoulder surgery may cause inferior shoulder arthroplasty outcomes, there is no consensus on whether previous rotator cuff repair (RCR) is associated with inferior outcomes after reverse total shoulder arthroplasty (RTSA).

PURPOSE:

To retrospectively compare outcomes in patients who underwent RTSA with and without previous RCR.

 

 

CONCLUSION:

Patients with previous RCR attempts may experience fewer short-term gains in functional and subjective outcome scores after RTSA compared with patients with no history of shoulder surgery who undergo RTSA. However, the differences between groups were small and below the minimal clinically important differences for the outcome measures analyzed.

ANTECEDENTES:
Aunque la evidencia reciente sugiere que cualquier cirugía de hombro previa puede causar resultados inferiores de artroplastia de hombro, no hay consenso sobre si la reparación previa del manguito rotador (RCR) se asocia con resultados inferiores después de la artroplastia total de hombro reversa (RTSA).

PROPÓSITO:
Comparar retrospectivamente los resultados en pacientes sometidos a RTSA con y sin RCR previa.

 

CONCLUSIÓN:
Los pacientes con intentos previos de RCR pueden experimentar menos ganancias a corto plazo en los puntajes de resultado funcionales y subjetivos después de RTSA en comparación con los pacientes sin antecedentes de cirugía de hombro sometidos a RTSA. Sin embargo, las diferencias entre los grupos fueron pequeñas y por debajo de las mínimas diferencias clínicamente importantes para las medidas de resultado analizadas.

Abstract

BACKGROUND:

Revision shoulder arthroplasty is a technically challenging procedure. It is associated with increased blood loss and operative time, and it frequently necessitates revision implants, augments, and bone-grafting. Shoulder arthroplasty systems with a convertible-platform humeral stem have been developed to reduce the complexity of revision procedures by eliminating the need for humeral component explantation when converting from anatomic shoulder arthroplasty (hemiarthroplasty or total shoulderarthroplasty) to reverse total shoulder arthroplasty (rTSA).

CONCLUSIONS:

Shoulder arthroplasty systems that utilize a convertible-platform humeral stem offer an advantage for rTSA conversion in that a well-fixed, well-positioned humeral stem can be retained. There were significantly fewer complications as well as significantly decreased blood loss and operative time when a convertible-platform stem was utilized (p < 0.050).

ANTECEDENTES:

La artroplastia de hombro de revisión es un procedimiento técnicamente difícil. Se asocia con aumento de la pérdida de sangre y el tiempo quirúrgico, y con frecuencia requiere implantes de revisión, aumentos y injertos óseos. Se han desarrollado sistemas de artroplastia de hombro con un vástago humeral de plataforma convertible para reducir la complejidad de los procedimientos de revisión eliminando la necesidad de explanación de componente humeral al convertir la artroplastia anatómica de hombro (ARTHA).

CONCLUSIONES:

Los sistemas de artroplastia de hombro que utilizan un vástago humeral de plataforma convertible ofrecen una ventaja para la conversión rTSA en que se puede retener un vástago humeral bien fijado y bien posicionado. Hubo significativamente menos complicaciones, así como una disminución significativa de la pérdida de sangre y el tiempo de operación cuando se utilizó un vástago de la plataforma convertible (p <0,050).
