Abstract

BACKGROUND:

Platform shoulder arthroplasty systems may allow conversion to a reverse total shoulder arthroplasty (RTSA) without removing a well-fixed, well-positioned humeral stem. We sought to evaluate the complications associated with humeral stem exchange versus retention in patients undergoing conversion shoulder arthroplasty with a platform shoulder arthroplasty system.

 

CONCLUSION:

Conversion arthroplasty with retention of the humeral stem is associated with lower overall complications, blood loss, operative time, and reoperations in comparison with stem exchange.

Resumen

ANTECEDENTES:

Los sistemas de artroplastia de hombro de plataforma pueden permitir la conversión a una artroplastia total de hombro inversa (RTSA) sin extirpar un vástago humeral bien posicionado y bien posicionado. Intentamos evaluar las complicaciones asociadas con el intercambio de vástago humeral versus la retención en pacientes sometidos a artroplastia de hombro de conversión con un sistema de artroplastia de hombro de plataforma.

CONCLUSIÓN:

La artroplastia de conversión con retención del vástago humeral se asocia con menores complicaciones generales, pérdida de sangre, tiempo quirúrgico y reintervenciones en comparación con el intercambio del vástago.

Abstract

BACKGROUND:

Gender differences may exist for patients undergoing shoulder arthroplasty. Limited data suggest that women may have worse preoperative disability and outcomes. Our objective was to determine whether gender influences preoperative disability and patient-reported outcomes after reverse total shoulder arthroplasty.

 

CONCLUSION:

Improvements in pain and range of motion were similar in men and women; however, men achieved higher ASES function and SF-12 Physical Component Summary scores. Women may be more functionally impaired on the basis of differences in activities of daily living. These results may be used to guide discussion about expected benefits after reverse shoulder arthroplasty.

Resumen

ANTECEDENTES:

Las diferencias de género pueden existir para pacientes sometidos a artroplastia de hombro. Datos limitados sugieren que las mujeres pueden tener una peor discapacidad y resultados preoperatorios. Nuestro objetivo fue determinar si el género influye en la discapacidad preoperatoria y los resultados informados por los pacientes después de la artroplastia total de hombro invertida.

CONCLUSIÓN:

Las mejoras en el dolor y el rango de movimiento fueron similares en hombres y mujeres; sin embargo, los hombres lograron una mayor función de ASES y puntajes de SF-12 Physical Component Summary. Las mujeres pueden tener un deterioro funcional mayor debido a las diferencias en las actividades de la vida diaria. Estos resultados pueden usarse para guiar la discusión sobre los beneficios esperados después de la artroplastia de hombro invertida.

Abstract

OBJECTIVE:

Stabilization of the humerus with preservation or restoration of the shoulder function.

INDICATIONS:

Always in the presence of a loose prosthesis. It may become necessary in conditions of poor bone stock and if osteosynthesis is not possible.

CONTRAINDICATIONS:

Noncompliant patients due to alcohol or drugs. Local infections.

SURGICAL TECHNIQUE:

The loose implant is removed using an extended anterior deltopectoral approach. After exploration of the fracture and extended soft tissue release, the glenoidal components are implanted with visualization and protection of the axillary nerve. A long stemmed implant is typically needed on the humeral side. It is anchored in the distal fragment over a length of about 6 cm. Soft tissue tension is crucial, especially with reverse shoulder arthroplasty.

POSTOPERATIVE MANAGEMENT:

Postoperatively, the affected limb is immobilized for 6 weeks on a 15° shoulder abduction pillow with active assisted movement therapy up to the horizontal plane. This is followed by gradual pain-adapted increases of movement, muscle coordination, and strength.

RESULTS:

In 17 patients with periprosthetic fractures of the humerus surgically treated in our institution, 4 underwent revision arthroplasty because of a loose prosthesis. No intra- or postoperative complications were observed. All fractures healed except one.

OBJETIVO:

Estabilización del húmero con preservación o restauración de la función del hombro.

INDICACIONES:

Siempre en presencia de una prótesis suelta. Puede volverse necesario en condiciones de existencia de hueso pobre y si la osteosíntesis no es posible.

CONTRAINDICACIONES:

Pacientes no cumplidores debido a alcohol o drogas. Infecciones locales

TÉCNICA QUIRÚRGICA:

El implante suelto se elimina utilizando un abordaje deltopectoral anterior extendido. Después de la exploración de la fractura y la liberación prolongada de tejido blando, se implantan los componentes glenoideos con visualización y protección del nervio axilar. Por lo general, se necesita un implante de tallo largo en el lado humeral. Está anclado en el fragmento distal en una longitud de aproximadamente 6 cm. La tensión de los tejidos blandos es crucial, especialmente con la artroplastia de hombro reversa.

MANEJO POSTOPERATORIO:

En el postoperatorio, la extremidad afectada se inmoviliza durante 6 semanas en una almohada de abducción del hombro a 15 ° con terapia de movimiento asistido activo hasta el plano horizontal. Esto es seguido por incrementos de movimiento, coordinación muscular y fuerza adaptados gradualmente al dolor.

RESULTADOS:

En 17 pacientes con fracturas periprotésicas de húmero tratadas quirúrgicamente en nuestra institución, 4 se sometieron a una artroplastia de revisión debido a una prótesis suelta. No se observaron complicaciones intra o postoperatorias. Todas las fracturas se curaron excepto una.

Abstract

BACKGROUND:

There has been serious concern regarding the longevity and durability of outcomes of reverse total shoulder arthroplasty (RTSA) in younger patients. It was the purpose of this study to analyze long-term outcomes and complications of RTSA for irreparable rotator cuff tears in patients younger than 60 years.

CONCLUSIONS:

RTSA in patients younger than 60 years leads to substantial subjective and functional improvement without clinical deterioration beyond 10 years. It is associated with a substantial complication rate, and complications compromise ultimate subjective and objective outcomes.

ANTECEDENTES:
Ha habido una gran preocupación con respecto a la longevidad y la durabilidad de los resultados de la artroplastia total de hombro reversa (RTSA) en pacientes más jóvenes. El objetivo de este estudio fue analizar los resultados a largo plazo y las complicaciones de RTSA para los desgarros irreparables del manguito rotador en pacientes menores de 60 años.

CONCLUSIONES:
La RTSA en pacientes menores de 60 años conduce a una mejora sustancial subjetiva y funcional sin deterioro clínico más allá de los 10 años. Se asocia con una tasa de complicaciones considerable, y las complicaciones comprometen los últimos resultados subjetivos y objetivos.
