Abstract

BACKGROUND:

Mental health conditions are associated with poor outcomes in patients with chronic disease as well as various orthopedic conditions. The purpose of this study was to describe the relationship between mental health, preoperative disability, and postoperative outcomes in patients undergoing shoulder arthroplasty.

METHODS:

Data, including mental health diagnoses, were prospectively collected from patients undergoing total shoulder arthroplasty or reverse total shoulder arthroplasty from 2009 to 2015 at a single academic institution. Shoulder range of motion, visual analog scale, 12-Item Short Form Health Survey, and American Shoulder and Elbow Surgeons scores were collected preoperatively and at 1 and 2 years postoperatively. Data were analyzed using multivariate mixed-effect regression analysis.

RESULTS:

The study included 280 patients, 105 (37.5%) of whom had a mental health diagnosis of depression, anxiety, schizophrenia, or bipolar disorder. Both groups of patients had similar shoulder range of motion, pain, and function before shoulder arthroplasty. Hospital length of stay, discharge destination, and readmissions were similar for both groups. There were similar improvements in pain, function, and range of motion after shoulder arthroplasty in patients with and without diagnosed mental health conditions.

CONCLUSIONS:

Overall, the presence of a psychiatric diagnosis was not predictive of outcomes. Although psychiatric conditions are often considered surgical comorbidities, mental health diagnoses should not be a barrier to performing clinically indicated shoulder arthroplasty, because both groups of patients appear to benefit from pain relief and improved shoulder function.

Resumen

 

ANTECEDENTES:

Las condiciones de salud mental se asocian con malos resultados en pacientes con enfermedades crónicas, así como en diversas afecciones ortopédicas. El propósito de este estudio fue describir la relación entre la salud mental, la discapacidad preoperatoria y los resultados postoperatorios en pacientes sometidos a artroplastia de hombro.

 

MÉTODOS:
Los datos, incluidos los diagnósticos de salud mental, se recogieron prospectivamente de pacientes sometidos a artroplastia total de hombro o artroplastia total de hombro invertida desde 2009 hasta 2015 en una sola institución académica. El rango de movimiento del hombro, la escala análoga visual, la Encuesta de salud de forma breve de 12 ítems y los puntajes de los cirujanos estadounidenses de hombro y codo se recolectaron antes de la cirugía y 1 y 2 años después de la operación. Los datos se analizaron mediante análisis de regresión multivariante de efectos mixtos.

 

RESULTADOS:
El estudio incluyó a 280 pacientes, 105 (37.5%) de los cuales tenían un diagnóstico de salud mental de depresión, ansiedad, esquizofrenia o trastorno bipolar. Ambos grupos de pacientes tenían un rango de movimiento, dolor y función similar al hombro antes de la artroplastia de hombro. La duración de la hospitalización, el destino del alta hospitalaria y las readmisiones fueron similares para ambos grupos. Hubo mejoras similares en el dolor, la función y el rango de movimiento después de la artroplastia de hombro en pacientes con y sin afecciones mentales diagnosticadas.

 

CONCLUSIONES:

En general, la presencia de un diagnóstico psiquiátrico no fue predictivo de los resultados. Aunque las afecciones psiquiátricas a menudo se consideran comorbilidades quirúrgicas, los diagnósticos de salud mental no deberían ser una barrera para realizar una artroplastia de hombro clínicamente indicada, ya que ambos grupos de pacientes parecen beneficiarse del alivio del dolor y la mejoría de la función del hombro.
